If you would like to request a user account for the Apex School-Based Health Center (SBHC) Data Hub or Apex Data App, please email support@apexeval.org or submit a ticket and include the following information:
The account owner's first and last name.
The account owner’s email address.
A list of sites the account owner will be accessing.
The desired permission level/role for the account. If you are unsure, please indicate the type of work the account owner will be performing.
Apex Support will review this information for accuracy and follow up with the project coordinator with next steps. Each new account owner will be emailed instructions for password creation. If you are experiencing problems with your invitation email please email support@apexeval.org.
When a user account first accesses the Apex Data App, the system will prompt the account owner to create a PIN when they first log in. This PIN will be used in place of the account’s password when logging into the App.
