Reports and Provider Reviews
The Apex Data system compiles all information collected from a Client Record (survey or screening tool) into an easy to read Client Record Report. This Client Record Report includes the client’s responses as well as graphs and icons displaying their results.
Once a client has completed their screening, providers may want to view their Client Record Report. The Client Record Report may be viewed in the Apex App or in the Apex School-Based Health Center (SHBC) Data Hub.
Once the Client Record Report has been reviewed, providers may want to complete a Provider Review. The Provider Review may be completed in the Apex App or in the SBHC Data Hub. A Provider Review allows providers to indicate what actions were taken with the client, what risks or resiliencies were noted, and provides an option to sign and date the Client Record, showing that it has been reviewed and by whom.
