LOG INTO THE APEX DATA APP

1. Log into the Apex Data App using your email address and password if it is your first time using the App on the device. If you have used this device before, select your email address and enter your six-digit PIN.

LOCATE THE CLIENT RECORD

2. Click the Records button on the right.

3. Find the Client Record in the list of records displayed.

   1. Hint: You can use the Search or filter options at the top of the page to help you find your client.

COMPLETE THE PROVIDER REVIEW

5. Click on the Actions menu icon at the far right of the Client Record.

6. Click Provider Review.

7. Use the survey section menu on the top right to navigate to the Provider Review sections.

   1. The Provider Review will be the last section of the survey.

8. Complete the Provider Review sections, being sure to answer every question.

9. Click Submit.
