      Screening Clients - Generating a survey in the SBHC Data Hub

      LOG INTO THE SBHC DATA HUB

      1. Using your email address and password, log into the SBHC Data Hub.

      CREATE A NEW CLIENT RECORD

      1. In the menu on the left side of the screen, click Clients to expand the menu.

      2. Click Records in the menu.

      3. Click the gray + Add New button

      4. Complete the following fields:

        1. Group

        2. Form (this is the survey)

        3. Language

        4. Invitation Expiry

        5. Client

          1. Search for an existing client or click + to add a new client profile.

        6. Email and/or Phone Number

      5. Click SAVE.

      A link to complete the survey will be sent to the email address and/or phone number provided.

      The client will need to click on the link to complete the survey.
