LOG INTO THE SCHOOL-BASED HEALTH CENTER (SBHC) DATA HUB

1. Using your email address and password, log into the SBHC Data Hub.

LOCATE THE CLIENT RECORD

2. In the menu on the left side of the screen, click Clients to expand the menu.

3. Click Records in the menu.

4. Find the Client Record in the list of records displayed.

   1. Hint: You can use the Search or filter options at the top of the page to help you find your client.

COMPLETE THE PROVIDER REVIEW

5. Click on the menu icon at the far right of the Client Record.

6. Click Take.

7. Use the survey section menu on the left to navigate to the Provider Review sections.

   1. The Provider Review will be the last section of the survey.

8. If you want to view the Record Report, click on View Report in the top section of the page.

9. Complete the Provider Review sections, being sure to answer every question.

10. Click Save and Exit.
