Anonymous screening options are available within the Apex School-Based Health Center (SBHC) Data Hub and the Apex Data App.

Please note that any screening tool or survey can be administered anonymously via the SBHC Data Hub. Only specific surveys designed to be anonymous, such as the SBHC Client Feedback Survey, can be administered anonymously in the Apex App.

ANONYMOUS SCREENING - APEX APP

GENERATE A NEW SURVEY RECORD

1. Once logged into the Apex App, click Start a Form.

2. Select an anonymous screening tool and click Next.

   1. NOTE: Only specific surveys designed to be anonymous, such as the SBHC Client Feedback Survey, can be administered anonymously in the Apex App.

3. Click Administer Forms and then Confirm to lock the device for form administration.

4. Pass the device to the client for them to complete the anonymous screening.

   1. NOTE: Client Records for surveys that are designed to be anonymous will not appear on the Client Records page.
